Yes, the narrator we have trusted the entire time is actually guilty.

He killed Roger Ackroyd to hide the fact that he was the one blackmailing Mrs. Ferrars.

When Mrs. Ferrars confessed to Roger, Dr. Sheppard knew his secret would come out so he murdered Roger to protect himself.

This twist completely changes how readers think about the story.
