Proactive Health is a new primary care practise that works with patients to create a tailored health and wellness plan based on six basic health pillars

After many years as internal medicine doctors, we've discovered that patients often only see their doctor to solve a pressing problem. As a result, we saw the need to create a new primary care practise that engages each and every patient, provides complete health tools and services, and alters the patient experience.

Carolinas HealthCare System's Proactive Health is a novel approach to primary care. For coaching and support, our team offers a diverse group of specialists, including physicians, a nurse practitioner, a qualified dietician, registered nurses, and health advocates. Each patient completes an unique and comprehensive health evaluation with our team in order to set health goals that are aligned with six essential health pillars:

1.Medical

It's critical to develop and maintain a connection with a primary care physician who will work with you to better understand your medical risks and analyse your overall health. In order to avoid the health implications of untreated chronic medical disorders, preventive care is critical. It's also critical to effectively identify and manage chronic medical issues. You may improve not only the length but also the quality of your life by getting the correct health services and testing.

2.Fitness

Regular exercise keeps your body mass index in a healthy range, lowering your risk of heart disease and diabetes, as well as combatting depression, osteoporosis, and dementia. Exercise is essential for the treatment of high blood pressure, joint and muscle problems, and stress relief. Physical activity is important for both the body and the mind, which is why we start by measuring your current fitness level and working with you to develop activity objectives.

3.Sleep

Sleep is the immune system's main support mechanism, and it is extremely necessary for the body and mind to function normally. The average American gets about 6.7 hours of sleep per night, despite the fact that research indicates that we require closer to 8 hours. We work with our patients to identify any health conditions that may be exacerbated by insufficient sleep and devise particular solutions to address these issues so that they can sleep as peacefully and restfully as possible.

4.Nutrition

Many of the vital elements our bodies require are stripped out during packaging in the ordinary American diet. Poor eating has now been linked to fatigue, headaches, insomnia, gastrointestinal issues, and even some malignancies. It's critical to get back to the basics. We can assist you in developing a new way of eating that is not only beneficial to your overall health but also enjoyable, simple, and delicious. Our patients have access to app-based tracking tools, videos, recipes, and more in addition to the counsel and support they will receive from our registered nutrition professionals.

5.Mindfulness

The state of mindfulness is described as being aware of and accepting of one's current situation. Mindfulness-based practises are now known to aid in the treatment of chronic illnesses as well as general mental health. We believe that mindfulness is a vital cornerstone of every patient's wellness journey, given the ongoing demands and stressors of daily living that many people experience. Our advice and resources can assist our patients in finding greater happiness and calm in their daily lives.

6.Social Health

Social wellbeing is crucial not just for mental health, but also for physical health. Meaningful relationships and support systems are well-known. We believe that by knowing and supporting our patients' social requirements, we can create a better overall experience at Proactive Health.
