She is taken into custody, faced with a court trial, and afterward placed into a safeguarded psychological ward called The Grove, where she refrains from speaking.

Theo Faber: The Psychotherapist Who Makes Things Personal
At this juncture, we are introduced to Theo Faber, a criminal psychotherapist who is inexplicably drawn to Alicia’s story. From a safe distance, Theo has been reading about Alicia’s case staring at her mute face, thinking to himself how much he can cure her. So when a position becomes vacant at The Grove, Theo leaps at the chance to work with Alicia, certain that this time he will be able to demystify her silence as well as the murder of her husband.
Theo, as he continues to work on Alicia, becomes obsessed with the case. Some aspects of Alicia’s life resonate with him, and the more he learns about her, the more he seeks to know.
In his sessions with Alicia, silence is the response that Theo receives. Still, he does not yield. In the process, Theo manages to reconstruct the chain of events that led to the night of Gabriel’s murder.
Understanding Alicia
Alicia’s character is more multifaceted than the surface suggests, Theo finds out as he digs deeper in the investigation. She talks to many people who at one time knew Alicia and reads their primary sources – her diaries. Alicia had a difficult past, as we learn about her due to those who knew her. There was little love between Alicia and her father. Such deep wounds could be inflicted only in early childhood, which obviously remained with the adult Alicia.
The days prior to Gabriel’s demise, Alicia's diary shows she was increasingly becoming paranoid. She mentions the distinct possibility of being stalked, and that she was increasingly suspicious of Gabriel’s motives. This gives her readers insight into the woman’s emotions but begs lots of questions. Like, was anyone watching Alicia, or was it just inside her head? Was her relationship with Gabriel all rainbows and butterflies or were there underlying issues?
Because these disclosures help, we begin to appreciate Alicia, not as a killer who has chosen to remain mute after the crime, but as a woman with emotional scars that she struggles to deal with. Instead, it is how she chooses to deal with the pain sustained and in turn how the truth is evaded.
The Painting: Alicia’s Silent Cry for Help
The Silent Patient has many elements at its core but one of the most interesting remains the painting that Alicia creates following the murder of her husband. Alicia adapted the painting and gave it her own title: Alcestis. The myth relates to the wife of Admetus who when asked to die for her husband’s sake went to the underworld and forever associated herself with silence.
The painting in the story takes on particular significance which drives the story. It's the only thing she painted after the murder, and Theo thinks he knows why she murdered Gabriel, and it is in the painting. He notices that the longer he looks at the painting, the more lossy it becomes and reflection starts to paint the aperture of Alicia herself. Is she, like Alcestis, a woman willing to put herself on the line for others? What is she saying by not saying anything? Or is there more to it, like a shelter?
Alicia's character continues to be a little more complicated with the introduction of this painting and her silence is intriguing.
The Unraveling Mystery: Can Theo Get Alicia to Speak?
During the course of the novel, Theo always makes it a point to speak to Alicia. He employs all sorts of therapeutic measures and also seeks to learn about her life outside The Grove, but to no avail. With each passing stage, Theo draws nearer to the truth. However, it can be observed that it is the distance from the truth A to B that poses the greatest risk, to both himself and Alicia.
One of the most compelling aspects of The Silent Patient is the way it plays with the reader’s expectations. Just when you think you have the mystery figured out, the novel throws in new twists and turns that keep you guessing. Theo’s investigation leads him down a path filled with deception, secrets, and shocking revelations. The book builds to a climax that will leave readers stunned, but without revealing any spoilers, it’s safe to say that the ending is both surprising and satisfying.
