The first error is not having a calorie deficit. If you don't have a calorie deficit, no matter how "clean" you eat or how much you exercise, you'll never lose weight. You will gain weight if you consume more calories than you expend each day. You'll lose weight if you eat less calories than you expend. It is a proven scientific truth.
