In 2600 BC, Ahk-Ton, the dictatorial ruler of Kahndaq, formed the Title of Sabbac, which was known to confer the person who wears great power. After trying to stage a rebellion, the ancient Council of Wizards gives the powers of Shazam on a young slave boy, turning him into Kahndaq's brave hero; the hero allegedly kills Ahk-Ton and stops his realm.
Kahndaq is now tormented by the Intergang. Adrianna Tomaz, an archaeologist, tries to uncover the Crown of Sabbac with the help and support of her brother Karim and coworkers Samir and Ishmael Gregor. Intergang attacks them as Adrianna gets the crown. Adrianna says a spell that awoke Teth-Adam, whom she believes to be Kahndaq's hero; Adam whereupon tortures and kills the majority of the Intergang troops.
Amanda Waller, a public servant, realises the event and links Hawkman and the JSA (Justice Society of America) (consisting of Doctor Fate, Cyclone, and Atom Smasher) for assistance in apprehending Adam. The JSA arrives just in time to prevent Adam from wreaking further havoc on the city and Intergang's allied forces. The JSA informs Adrianna that Adam was not a redeemer in his day, but rather a psycho who was jailed rather than buried.
Ishmael is confirmed to be Intergang's leader, and he pursues Adrianna's teenage son Amon, who has taken away the crown. Adam, Adrianna, and the JSA plan to exchange the crown for Amon. They contact Ishmael, who uncovers that he is the very last descendant of King Ahk-Ton and requests the crown to regain his rightful place on the throne, which Adrianna gladly gives and saves Amon's life. Ishmael did break his end of the deal and tried to shoot at Amon, having caused Adam to lose all control and dismantle the hiding place with his energy, killing Ishmael.
Hawkman and Adam both fled to the ruins of Ahk-Ton as a result of their guilt. He informs the latter that inaccurate stories about Kahndaq's champion have been circulated; Hurut, Adam's son, was actually the one who received Shazam powers and became Kahndaq's champion. Ahk-assassins Ton's were given the order to kill Adam and Hurut's mother since they knew that Hurut was invulnerable. Hurut, who thinks Adam will be the saviour Kahndaq needs, grants Adam the Shazam abilities when he discovers him severely wounded. The moment the now-mortal Hurut is killed by Ahk-assassins, Ton's Adam becomes furious and kills all the king's warriors, accidentally bringing doom to Kahndaq.
Adam is then taken to the Rock of Eternity by the Council of Wizards and held there. Adam turns himself in to the JSA and is taken to a Task Force X black site because he feels he is not capable of being a true hero. Soon after, Fate notices an unsettling vision of Hawkman's approaching demise. As the gang makes their way back to the city, they realise Ishmael forced Adam to kill him while he was wearing the Crown of Sabbac in order for him to be resurrected as the host of the demon Sabbac, who then emerges from the underworld to claim his reign.
Sabbac unleashes the Legion of Hell to torment Kahndaq, but Amon rallies the populace alongside Adrianna and Karim to drive them back. In Ahk-ruins, Ton's the JSA is getting ready to face Sabbac, but Fate summons a magical force field that prevents Hawkman, Cyclone, and Atom Smasher from entering while simultaneously revealing that Hawkman's demise can be prevented through his own sacrifice. Then Fate battles Sabbac alone while simultaneously releasing Adam via astral projection. When Sabbac kills Fate, the force barrier is broken and the others are free to engage in combat. Adam shows up just as Sabbac is about to murder the JSA and engages him, killing him. Adam accepts his new position as the guardian and king of Kahndaq and the JSA part ways amicably with him. He also takes on the new name Black Adam.
Waller tells a defiant Adam not to leave Kahndaq, but Superman then shows up and proposes the two of them have a conversation.
