Men’s pelvic floor health is an unknown and taboo subject in the medical field, as well as among patients. It remains largely unrecognized, until you start experiencing symptoms.

My name is Julien Trapeau, and I am a physiotherapist with specialized training in men's pelvic health. My journey into this field began with a personal struggle, one that many men experience but few talk about. I developed prostatitis, a condition that caused significant discomfort and disrupted my daily life. I consulted multiple doctors who prescribed medication, which provided temporary relief. However, no real long-term solutions were offered.

Women’s pelvic health is a more recognized subject, although still lagging behind compared to other areas of medicine. In my opinion, women's health is unfortunately undervalued in our medical system. However, men’s pelvic floor health is an even more unknown subject within the medical community as well.

This realization inspired me to train in pelvic floor physiotherapy specifically for men, ensuring that male patients had access to the care they needed. Now, as a physiotherapist with expertise in this area, my goal is to help men regain control, confidence, and quality of life through evidence-based treatment.

Breaking the Taboo in Men's Health

Men's pelvic health remains a largely unspoken topic, both among patients and within the healthcare community. Many men feel embarrassed to discuss symptoms related to their pelvic floor, often considering them to be part of aging. Many healthcare providers lack the training or awareness to address these issues, further reinforcing the stigma and leaving men with little to no guidance. By breaking the silence and encouraging open discussions, we can promote better care, improved outcomes, and greater awareness of men's pelvic health.

Understanding Men's Pelvic Floor Physiotherapy

Men's pelvic floor physiotherapy is a specialized area of physiotherapy focused on assessing and treating the muscles and nerves of the pelvic region. The pelvic floor muscles play a crucial role in bladder and bowel control, sexual function, and core stability. When these muscles become weak, tight, or uncoordinated, it can lead to a range of issues, including pain (pelvic, hips, or low back), incontinence, and erectile dysfunction.

Treatments are designed to help men regain strength, flexibility, and function in the pelvic area, in order to improve their overall quality of life. Whether the issue stems from surgery, chronic tension, or other medical conditions, pelvic floor physiotherapy provides evidence-based solutions to deal with symptoms.

Do I Need Men's Pelvic Floor Physiotherapy?

Physiotherapy treatment could be beneficial if you experience any of the following:

  • Urinary incontinence (leakage or urgency)
  • Post-prostatectomy recovery
  • Erectile dysfunction
  • Pelvic pain
  • Pain with sexual intercourse
  • Chronic constipation or bowel dysfunction
  • Persistent low back pain, hip pain, or groin pain
  • Chronic Prostatitis

These concerns can significantly impact daily life. It’s not fun to have to stay close to a bathroom at all times, or to wake up at night more than twice (Yes, more than two bathrooms trip at night is not normal and it should be treated). These conditions are not something you must just accept. Pelvic floor physiotherapy can effectively address these problems, helping most men fully return to their regular activities and improve their quality of life.

What Can I Expect?

A physiotherapy assessment for men’s pelvic health involves a session with a specially trained physiotherapist. You will be asked about your bladder and bowel function, sexual health, and other relevant symptoms. Assessment and treatment may include a detailed evaluation of the pelvic floor muscles, potentially including an internal rectal examination to determine the causes of your symptoms.

A rectal examination is usually a must, since it is kinda the only way to accurately test the muscle. Your physiotherapist will explain everything in detail before they proceed.

Based on your individual goals, your physiotherapist will create a personalized treatment plan, which may include:

• Manual therapy: Hands-on techniques to improve muscle function and reduce pain.

• Tailored exercise programs: Strengthening and coordination exercises specific to your needs.

• Education: Guidance on managing your condition and long-term strategies for pelvic health.

Your physiotherapist will work with you to develop a personalized treatment plan aimed at restoring your strength, control, and confidence in daily life.

How to Find a Physiotherapist Trained in Men's Pelvic Health

Finding a physiotherapist trained in men's pelvic health can be challenging, as this specialization is not widely known. Most family doctors may not be familiar with pelvic floor physiotherapy for men, and while some urologists may be aware of its benefits, they may not always refer patients. Therefore, doing your own research is crucial.

To find a qualified physiotherapist:

  • Look for physiotherapists with training and certifications in pelvic health, particularly those who specifically mention treating male patients.
  • Search for clinics that explicitly offer men's pelvic health services.
  • Don’t hesitate to contact physiotherapists directly to ask about their experience and approach to treating men’s pelvic health concerns.
