By the year 2043, a deadly virus of unknown origin has wiped out 7 billion people. Only 1% of the human population remains, struggling to survive in underground bunkers. Among these survivors is James Cole, a lone wanderer in the post-apocalyptic wasteland.

One day, Cole and his friend stumble upon the abandoned offices of the Center for Disease Control (CDC). While searching through the ruins, Cole retrieves a broken watch from the skeletal remains of a scientist. They soon return to a secret location known as Project Splinter, where a team of scientists, led by Dr. Jones, has spent three decades perfecting a time machine. Their mission is clear: send someone back in time to stop the virus before it spreads.

Cole, knowing the world is beyond saving unless the past is altered, volunteers for the mission. Before departure, Dr. Jones injects him with a special serum to help his body withstand time travel. Moments later, Cole steps into the massive machine and is sent back to the year 2015. His objective: locate Dr. Cassie Railly, a CDC scientist with key information about the virus that will ravage the world in 2017.

However, a malfunction sends Cole to 2013 instead. Undeterred, he tracks down Cassie at a medical conference, where she is presenting research on communicable diseases. As she leaves, Cole appears in her car’s back seat, startling her. He forcefully demands her cooperation, explaining that he is from the future and that they must work together to prevent global catastrophe.

Cassie, naturally, is skeptical and assumes he is delusional. To prove his claims, Cole presents the broken watch from 2043, identical to the one Cassie is wearing. When he scratches her watch, an identical mark appears on the future version. Stunned, she begins to believe him.

Cole explains that a man named Leland Goines is responsible for creating the virus. If they eliminate him, they can alter history. But Cassie has never heard of Goines, making Cole realize she doesn’t yet possess the crucial information. He instructs her to meet him at the John Adams Hotel in two years before vanishing into thin air.

Two years later, Cassie, now disillusioned and having lost her career and relationships due to her obsession with Cole’s warning, waits for him at the hotel. Just as she begins to lose hope, Cole reappears. He tells her that in 2017, the virus will spread worldwide, killing millions daily. Cassie, as the CDC’s chief scientist, will search for a cure but will ultimately succumb to the disease. Before dying, she will record a message revealing that Leland Goines was behind it all.

Determined, Cassie has spent the past two years researching Goines and discovered he is the CEO of a biotech company producing biological weapons. She also knows he will be attending a gala that night. Using her connections, she secures entry for both herself and Cole.

At the event, Cassie distracts her ex-boyfriend Aaron, who is also present, while Cole searches for Goines. As he moves in to assassinate him, Goines’ guards spot Cole’s weapon and capture him. Cassie is arrested as well, and both are taken to a secret laboratory.

There, scientist Peter examines Cole, fascinated by his brain's abnormal activity. Goines, having encountered Cole in 1987, reveals that he has been expecting him. Confused, Cole insists he has never time-traveled to 1987, but Goines insists otherwise, mentioning the "Army of the 12 Monkeys," a mysterious organization set to destroy the world.

With no time to waste, Cole devises an escape plan. He takes Cassie’s present-day watch and presses it against his future watch, creating a temporal anomaly. The resulting energy explosion disrupts the lab, allowing them to break free. Amid the chaos, Cole shoots Goines, believing he has successfully prevented the apocalypse.

However, Cassie soon points out a chilling realization—if Cole truly changed history, he should have ceased to exist. His continued presence suggests that the outbreak is still destined to happen. With time running out, Cole urges Cassie to flee before he vanishes back to 2043.

Back in the future, Cole reports to Dr. Jones. Despite killing Goines, the virus still emerged, meaning someone else released it. As they investigate further, they discover a disturbing clue—an old news report featuring a strange monkey symbol, linking it to the shadowy "Army of the 12 Monkeys." Additionally, they refine Cassie’s recorded message from 2017 and find that she also mentioned this enigmatic group.

Determined to uncover the truth, Cole travels back once more, this time to infiltrate a mental institution where Jennifer Goines, Leland’s daughter and a key witness, resides. However, before he can extract any useful information, a masked group led by a sinister figure known as "The Pallid Man" kidnaps Jennifer. The leader claims to have a scar from a past encounter with Cole—an encounter Cole has no memory of.

With the enemy always one step ahead, Cole realizes that stopping the virus is far more complicated than he ever imagined. The past is full of hidden players, and the true orchestrators of humanity’s downfall are still lurking in the shadows. But one thing is clear—time is running out, and the fate of the world rests on Cole’s ability to untangle the web of deception before history repeats itself.
