
Chapter 1: The Arrival of Mr. Harvey
Dr. Emily Tan sat in her office, scrolling through the day’s appointments. Her first patient was Mr. Harvey, a retired teacher in his late sixties who’d recently been referred to her for a persistent cough and fatigue. When he entered, Dr. Tan noted his hopeful smile mixed with a look of desperate anticipation—a look she’d come to recognize.
“Good morning, Mr. Harvey. What brings you in today?” she asked warmly.
He shifted in his seat. “It’s this cough, Doctor. It’s been going on for months now. I’ve tried all sorts of things, but nothing works. I was told you’d figure it out. They said you’d know what’s wrong.”
Dr. Tan nodded, trying to exude calm confidence. “Let’s see what we can do to get to the bottom of it.”
This wasn’t the first time a patient had assumed she had all the answers, and it wouldn’t be the last. She ordered an X-ray, ran blood work, and took a thorough history, listening carefully as Mr. Harvey described his worsening fatigue and breathlessness. But as she reviewed his symptoms, she felt a gnawing worry. The tests were routine, but the results might not be straightforward.

Emily remembered medical school all too well. She’d entered bright-eyed and eager, ready to learn how to “fix people.” From day one, however, the pressure was intense. Professors warned them about burnout, how endless memorization would be their new normal, how patients would expect miracles, even from new doctors.
Yet she and her classmates were taught to present themselves as calm and collected, even if they were barely holding it together. Every wrong answer during rounds felt like a personal failure. By graduation, they’d absorbed the silent message: Never let anyone see you doubt, especially not a patient.
That lesson weighed on her now, as she examined Mr. Harvey’s inconclusive results.

Days later, Mr. Harvey was back, his symptoms growing worse. Dr. Tan ordered more tests, scouring his records for something they might’ve missed. But each test only ruled out a different condition, leaving his worsening cough and fatigue an unsolved mystery.
“Doctor, isn’t there something you can do?” he asked, his voice laced with frustration. “You’re the expert here.”
Emily felt a pang of guilt. How could she explain that expertise didn’t mean omniscience? Medicine, like any science, was full of trial and error, educated guesses, and sometimes more questions than answers.
“Mr. Harvey,” she said gently, “we’re doing everything we can. Some conditions just take a little more time to diagnose.”
He nodded, but she could tell he wasn’t satisfied. And she knew he wasn’t alone; patients expected doctors to have answers, a quick fix, or at least a name for what ailed them. What they didn’t see were the hours spent in uncertainty, the consultations with colleagues, and the occasional reliance on luck.

Emily decided to consult Dr. Alvarez, an experienced pulmonologist. Dr. Alvarez had a reputation for solving difficult cases and had been her mentor during residency.
“You’re not responsible for knowing everything, Emily,” he reminded her over coffee. “Patients come to us for guidance, not perfection. And sometimes, the best answer we can give is that we’re still looking.”
“But they want answers now,” Emily replied, feeling the familiar weight of responsibility.
“They always do,” he said, chuckling. “But being honest about our limitations doesn’t make us any less capable. It makes us human.”
The advice resonated with her, but she wondered if Mr. Harvey would feel the same.

At his next appointment, Emily decided to have a frank discussion with Mr. Harvey. She explained how his case was complex, and while they’d ruled out many possible causes, it might take more time and collaboration to identify the exact issue.
“But you’re saying… you don’t know what’s wrong with me?” he asked, a hint of betrayal in his voice.
“Not yet,” she admitted, holding his gaze. “But I promise, we’re working on it. I’ll consult with specialists, keep running tests, and do everything I can to get you the answers you need.”
Mr. Harvey’s face softened, though the frustration lingered. “I guess I thought… well, I thought doctors knew everything.”
Emily felt a small surge of relief. This was the opening she’d hoped for.
“That’s a common myth,” she said, smiling. “We’re here to help and guide, but sometimes, the human body is more complicated than textbooks and tests. It doesn’t mean we’re not working for you, though.”
For the first time, Mr. Harvey nodded with understanding.

Weeks later, after collaborating with specialists and conducting numerous tests, they finally diagnosed Mr. Harvey with a rare autoimmune condition. The journey to the answer hadn’t been easy, but it was worth it. Mr. Harvey began treatment and, though his road to recovery was still long, he thanked Emily profusely for her dedication.
“This whole experience,” he said, “it taught me something. I thought doctors had to know everything, but maybe the most important thing is that you never stopped trying.”
Emily smiled, realizing that both she and Mr. Harvey had learned something valuable: that the role of a doctor wasn’t to always have the answer, but to work tirelessly and honestly in search of it.
